1 Name of Instrument
(1) This Instrument is the National Health (IVF/GIFT Program)
Special Arrangement Amendment Instrument 2014 (No.1).
(2) This Instrument may also be cited as PB 58 of 2014.
2 Commencement
This Instrument commences on 1 August 2014.
3 Amendment of PB 93 of 2011
(1) Schedule 1 amends the National Health (IVF/GIFT Program) Special Arrangement 2011 (PB 93 of 2011).
Schedule Amendments
[1] Schedule 1, after the entry for ‘Progesterone’, in the form ‘Vaginal Gel (prolonged release) 90 mg in single dose pre-filled applicator’
insert:

[2] Schedule 3, Treatment Condition Item 1
substitute:
1 | A patient who: (i) Has an antral follicle count of 20 or less; and (ii) Is undergoing a gonadotrophin releasing hormone antagonist cycle | Corifollitropin Alfa | Solution for injection 100 micrograms in 0.5 mL single dose pre-filled syringes |
