This instrument revokes the Safety, Rehabilitation and Compensation Act 1988 - subsection 34S(1) - Approved Form for Application for Initial Approval as a Rehabilitation Program Provider (29/09/2006) and provides the approved form for the purposes of persons applying as a rehabilitation program provider.
